• Gender (★): Male / Female.

• Date of birth (★): Day / month / year. Example: 17/08/2017.

• Nationality (★)

• Grade applying for (★): Match your last promotion certificate.

• Previous school: Name and last grade.

• Health / allergies: Food or drug allergies and regular medication.

• School transport: Required / not required (if available).

• Preferred language or track (if any): Arabic / English / bilingual.

Parent / guardian

• Full name (★)

• Relationship (★): Father / mother / legal guardian.

• National ID (if any)

• Phone (★): Active number (e.g. WhatsApp). Example: 09XXXXXXXX.

• Email (★): For notifications.

• Job and workplace: For emergencies.

• Home address (★): City, area, street, landmark.

• Alternate emergency contact: Name, phone, relationship.

Uploads (★)

• Birth certificate, parent ID, photo, latest transcript, immunization (if needed), proof of address.

• Images must be fully readable, not cropped at the edges.

Declarations

• I confirm that all data and documents are true.
